Please upload the applicable documents listed below with your Hickam Communities application. If you don’t have all of your documents at the time of your application, you can still apply now and then upload the necessary documents here after your application is submitted. For Active Duty Service Members:
-Military Orders
-Flight Itinerary for service member and all family members
-Most recent end of month LES
-(If applicable) Leave form, DA31, Page 2, Surf (E1-E3)
For Non-Active Duty Applicants:
-Hickam Communities Leasing Application
-DD214, Orders or SF50, and/or future DoD Hawaii Job offer final letter (stating acceptance)
-Proof of Income, retirement account summary and/or future DoD Hawaii Job offer l final letter (stating acceptance)
*A credit and background check is required for all non-active duty applicants. There is a $30 fee per applicant for any person over the age of 18; a leasing consultant will contact you to collect payment. This is usually not completed until a MHO referral is completed and a home has been offered.
ALL APPLICANTS REQUIRE A COMPLETED MILITARY HOUSING OFFICE (MHO) REFERRAL PACKET:
-The MHO referral packet is a required document that qualifies an applicant’s eligibility to live in a Hickam Communities home. Please see the fillable MHO Referral forms, the MHO contact information and the MHO referral process below, to get a head start on obtaining your MHO referral packet.
The Military Housing Office (MHO) Referral Packet & Process:
1) The MHO Referral process can be initiated if you are already on island or arriving on island within the next 30 days.
2) Download & Complete the MHO referral packet - Please download (on to your device) the appropriate MHO referral packet below and complete all applicable fields /signatures/initials in Sections 1-3 (pages 1-6). The completed packet can be uploaded via the DoD Safe Site (see below) or dropped off in person to the MHO Office located at 200 Kokomalei Street, Honolulu, Hawaii 96818.
3) Schedule your MHO referral brief - Please contact the Military Housing Office at jbphh.mho@navy.mil or call 808-312-0199/808-590-1402 to schedule your MHO referral brief. Briefs are conducted Monday-Friday at 9am & 1pm Hawaii time. You may complete the MHO referral brief prior to completing your MHO referral packet, if desired.
MHO Referral Packets: Download (on to your device) the appropriate referral packet below by right-clicking on the link and selecting “Save link as”. Open the saved file in Adobe Reader in order to complete. If you do not have Adobe Reader, CLICK HERE to download it for free.
**Additional Information: If your dependent(s) is/are not included on your PCS orders, additional supporting documentation may be required such as: DEERS Verification of Dependents (DD Form 1172-2) from MPF, Pg 2, marriage or birth certificate, pregnancy verification with expected delivery date, 60-day prior to marriage letter signed by Commander or First Shirt, custody paperwork, etc.
If you wish to upload your completed MHO referral packet via the DoD SAFE Site, please follow the instructions below:
- Go to https://safe.apps.mil/
- CAC Card Users:
- Click “Drop-off”
- Add Recipient information:
- “MHO Referral” under Name
- Add “jbphh.mho@navy.mil" Under email address
- Add any additional recipients
- Click “Add and Close”
- Add “Short Note” with your Full Name, Phone and Email
- Click “Encrypt file” (Required)
- If desired, click “Send me an email when each recipient picks up the file” (Optional)
- Add, Drag or Drop Files to Upload
- Click “Drop-Off Files”
- Click “Ok” to confirm the files do not contain classified information
- You will be notified “Drop-Off Completed”
- The recipient (MHO) will be notified of your “Drop Off”
Guest Users:
- Email or call MHO to obtain a drop off file link - jbphh.mho@navy.mil or call 808-312-0199/808-590-1402
- You will receive an email with the drop off file link
- Click on the link
- An email back to MHO will auto populate on your screen
- Add, Drag or Drop Files to Upload
- Add a description of each file, if necessary
- Click “Encrypt every file (Required for PII/PHI)”
- Create a Passphrase, click ok
- Add “Short Note to the Recipient” with your Full Name, Phone and Email and PASSPHRASE (so that MHO can retrieve your files)
- Click "Drop-Off Files"
- Click “Ok” to confirm the files do not contain classified information
- You will be notified “Drop-Off Completed”
